Incident Report Form

Submit your findings for a free, confidential forensic assessment. It is strongly recommended that you review “How to Process a Scene” before proceeding, as you must check a box below confirming your adherence to the documentation checklist. Please provide the following to match the form fields: contact information; discovery details (including precise date and time of discovery, the last known time the animal was seen alive and healthy, and specific weather conditions during both events plus any interim environmental changes like rain, frost, snow, high winds, or temperature swings); precise GPS coordinates and distances; animal details (species, breed, age, sex, health before death, injuries observed, blood on the scene present or absent, and other physical signs); original photograph files (attach high-quality images directly in the form or, if the file size is too large, paste your download links in the “Evidence and file links” box); scene observations (including any suspicious or unusual activity in the area over the last week and noting if you have had previous mutilation incidents on the property); and law enforcement reports (attach official documents and letters in the “Supporting documents” upload field or summarize the relevant case details in the text fields); you may also include extra notes. For large attachments, paste download links directly into the “Evidence and file links” box.
